School-Sport Physicals and Well Visit Reminder

Summer is in full swing with days filled with parks, pools and vacations! Remember to add Bloomington Pediatrics to your list of summer activities and come see us for your well visit, school and sports physicals! We have openings available the rest of June and throughout July and August to make sure you get the appointment time and forms you need!

Routine examinations provide the best opportunity for the doctor to observe the progress of your child's physical and mental growth and development, to counsel and teach parents, to detect problems through the physical examination and screening tests, to provide immunizations, and to get to know one another. Well-care visits are strongly recommended as part of preventive pediatric care and are also a good time for parents to raise questions and concerns about a child's development, behavior, nutrition, safety and overall well-being.

The American Academy of Pediatrics recommends routine well-care visits once every year for an annual health supervision visit that includes a physical exam as well as a developmental, behavioral, and learning assessment. (Please be aware that some insurance plans will cover well checks only once every 12 months while others will allow one each calendar year.)

Physicals are required for children entering Kindergarten, 6th and 9th grade.

A physical is also required for children playing sports.

Your child may also need a physical form for certain summer camps or Boy/Girl Scout activities.

Call us at 309-662-0504, option 3 to secure a day and time that works best for you. Then check this off your to-do list and enjoy the summer fun!

Please check with your school, athletic department, parks and rec program and pack/troop leaders for any forms your child may need signed by their physician. We have most forms in the office and will do our best to get those signed and to you quickly!
