Our Practice is excited to offer a new, streamlined way for parents to complete developmental questionnaires ahead of their appointments, saving you time in the office completing paperwork!

 

Age appropriate questionnaires will be auto-assigned to the patient for completion up to 2 weeks prior to their appointment in the office. Patient families will access their CHADIS questionnaires through the Patient Portal. Once you have logged into the Patient Portal, you will scroll to the section titled CHADIS Questionnaires.

 

 

If you do not have an orange Start Questionnaires button, no questionnaires have been assigned to you or the patient to complete. If you believe your child should have been assigned questionnaires to complete for their upcoming visit, please contact the office! Clicking the orange Start button will take you to the Chadis page with the questionnaires we have sent for completion. If you need to stop in the middle of a questionnaire, you may resume it at any time.

 

 

Completed questionnaires will be securely imported directly into the patient record for their physician to review! Not all questionnaires used by our practice are available to complete through CHADIS, and as with all technology, sometimes technical difficulties may arise. We appreciate your patience and understanding when an old-fashioned paper copy of a form needs to be completed in the office!

 

Please read below for some CHADIS FAQ’s, but feel free to contact us anytime with additional questions or concerns!

 

1. “What is CHADIS?”
 

CHADIS is an online system that delivers questionnaires that help your child’s doctor review the health and development of your child.  Using CHADIS before the visit will help your doctor take better care of your child. Mental health and other medical specialists also use CHADIS for their patients.
 

2. “What is the goal of the questionnaires?”
 

Your child's doctor uses the questionnaires to help determine if your child is developing on schedule and without health risks. These questions can help detect issues early before some signs are present.  When issues are caught early, children are able to receive help earlier and treatment is often more effective.  National expert guidelines for "best care" have increasingly revealed that the more detailed information about a child a doctor has, the more opportunities there are for early preventive action. However, when doctors spend valuable visit time attending to important developmental issues by asking more and more detailed questions, the time available to properly address the priority issues, and your own questions, is lost. CHADIS allows parents to set the agenda and also not overlook important details.

 

3. “When should I use CHADIS?”

 

You should complete CHADIS questionnaires according to the schedule discussed with your doctor. Many general pediatricians or family doctors will want you to use CHADIS before every well-child visit and many will use it for a variety of other special visits, such as for asthma or ADHD.

 

4. “How do I get started?”

 

You can get started by ensuring you have Patient Portal access for each child. Due to HIPAA, each patient must have their own Patient Portal, but all children in a family may be linked to a parent for a single portal login. If you are unsure if your child has a Portal, or you need help resetting your username or password, please contact the office and we will be happy to assist you!

5. “Who can help me with technical issues?”

 

Please contact the office if you have any trouble accessing or completing your questionnaires. We will do our best to troubleshoot with you or will seek additional assistance from CHADIS directly, as needed.

 

6. “Is CHADIS HIPAA Compliant?”

 

Absolutely! CHADIS is fully HIPAA compliant.

 

7. “What is HIPAA?”

 

HIPAA is the federal Health Insurance Portability and Accountability Act of 1996. The HIPAA Privacy Rule provides federal protections for individually identifiable health information held by covered entities and their business associates and gives patients an array of rights with respect to that information. At the same time, the Privacy Rule is balanced so that it permits the disclosure of health information needed for patient care and other important purposes.

 

8. “Who has access to my CHADIS information?”

 

Only your doctor and the staff in the office have access to your information.  You will use your personal Patient Portal to access the CHADIS questionnaires.  We will never supply this information to anyone.

 

9. “I have more than one child. Do I need multiple accounts?”

 

No. While each patient must have an individual patient record, all children in a family may be linked to a single Patient Portal account login which you will access to complete their specifically assigned questionnaires.
