School & Sports Physicals Reminder
Spring is here! Which means Summer is just around the corner!
Physicals are required for children entering Kindergarten, 6th and 9th grade.
A physical is also required for children playing sports.
Your child may also need a physical form for certain summer camps or Boy/Girl Scout activities.
Avoid the summer rush and schedule your child's appointment today!

Please check with your school, athletic department, parks and rec program and pack/troop leaders for any forms your child may need signed by their physician. We have most forms in the office and will do our best to get those signed and to you quickly!
