Vaccine Clinic Information
First and Second Dose

Our practice is pleased to be able to offer Covid-19 vaccination to our patient families. All vaccine clinics are dependent on vaccine availability from the McLean County Health Department.

 

At this time we have completed all drive-thru style clinics and are scheduling a limited number of Covid-19 vaccine appointments inside the office each day. Please call the office at 309-662-0504 if you would like to schedule an appointment.

 

If you have a Covid-19 vaccine appointment scheduled, please see below for important information and required paperwork BEFORE your visit. 

 

In preparation for your/your child's upcoming Covid Vaccination, please download, print and complete the attached Pre-Vaccination Registration/Checklist and review the Vaccine Fact Sheet. Completion of the checklist and review of the fact sheet is required before you may receive your vaccination. If you are unable to print the checklist and/or review the fact sheet, please reply to this email and advise. If time allows, we will mail the sheets to you or you may pick up at the office. Bring the completed checklist with you to your appointment. If you have questions or concerns about the checklist or fact sheet, please call the office at 309-662-0504 and select option 1 for the nurse.

If your child is aged 16-17 and will be coming to the appointment on their own, we will need to have a completed Consent to Treat an Unaccompanied Minor form on file. If you have not previously completed this form for your child, please click below to complete the online form. This must be signed by a parent/legal guardian.

On the day of your appointment, please follow our current check in process for in-office appointments.

 

After your vaccination, you will be given your CDC Covid-19 Vaccination Record Card and a timer and will be asked wait in our observation area where a nurse will be monitoring for any post-vaccine reaction. 

 

The above process will be the SAME for your 2nd dose appointment. However, you will need to bring the CDC Covid-19 Vaccination Record Card with you so we may document the 2nd dose. We will NOT be able to provide a second card, so please make sure you have it, and your Checklist for that appointment date with you when you come.
