COVID 19

Our practice is pleased to be able to offer Covid-19 vaccination to our patient families. Please read on for detailed information on our clinic dates, scheduling, required documentation, and processes.
*Effective 11-10-2022: we are no longer administering primary series or booster covid vaccines for ages 5+. Current ill patient volume and staffing do not allow us to continue to offer these at this time. We apologize for the inconvenience. These vaccines are widely available at most pharmacies and through the McLean County Health Department.
*Primary series in-office vaccines are still available for Moderna 6mo-4 yr .
We urge you to be patient when calling the office after clinic dates are announced. Our call volume is extremely high at these times and we do our best to answer as quickly as possible.
- ALL vaccines are by appointment only and individuals must be a patient of Bloomington Pediatrics
- Clinic dates will be posted on our Facebook page and here on our website.
- All vaccine clinics are dependent on vaccine availability from the McLean County Health Department.
If you have questions or concerns about the vaccine, please call our office at 309-662-0504 and select option 1 to speak with a pediatric nurse.
For all other Covid Vaccine appointment requests, please call the office at 309-662-0504 and select option 3 for scheduling.
REQUIRED PAPERWORK
If you have a Covid-19 vaccine appointment scheduled, please see below for important information and required paperwork BEFORE your visit.
In preparation for your/your child's upcoming Covid Vaccination, please download, print and complete the attached Pre-Vaccination Registration/Checklist and review the Vaccine Fact Sheet. Completion of the checklist and review of the fact sheet is required before you may receive your vaccination. If you are unable to print the checklist and/or review the fact sheet, please let us know. If time allows, we will mail the sheets to you or you may pick up at the office. Bring the completed checklist with you to your appointment. If you have questions or concerns about the checklist or fact sheet, please call the office at 309-662-0504 and select option 1 for the nurse.

APPOINTMENT DAY CHECK-IN PROCESS
Please follow our current check-in process for NON-CLINIC traditional in-office appointments.
We appreciate your cooperation with our nursing staff to administer the vaccine to our patients in the safest manner for all.
- Parents need to be prepared to assist in holding their child.
- Nurses have the authority to decline to administer the vaccine if your child is uncooperative.
The above process will be the SAME for your 2nd dose and/or Booster appointment. However, you will need to bring the CDC Covid-19 Vaccination Record Card with you so we may document subsequent doses. We will NOT be able to provide a second card, so please make sure you have it, and your Checklist for that appointment date with you when you come.
